I created my own report sheets 1) to keep myself organized while I was working in a Level IV NICU and 2) because all of the other ones online are too cluttered! I like more white space than pre-entered typing. I like a report sheet with some structure, but not too much structure AND I am not interested in only being able to fit one patient per page (OG nurses know that unless your kid is an extremely complicated case, 1 patient per page is absurd).
